“Geronte Consortium: Moving Closer to Proof of Concept for a New Model of Care

This year’s Geronte Consortium Meeting brought together all members of the consortium to reflect on progress, align priorities, and chart the next phase of our shared mission: to design and deliver a care model that meets the complex and evolving needs of older adults living with cancer and multimorbidity.

With significant advances – from the enhanced functionality of the Geronte app to the expanded training and integration of Advanced Practice Nurses (APNs) – we are now moving decisively towards demonstrating proof of concept. These developments are not just technological or clinical milestones; they represent a fundamental rethinking of how healthcare systems can respond to the realities of ageing populations.

The Geronte care pathway offers a scalable, evidence-informed framework that combines digital tools, multidisciplinary expertise, and patient-centred principles into routine practice. Beyond improving outcomes for individuals, it provides a strategic blueprint for health system transformation – showing how integrated care models can bridge gaps across oncology, geriatrics, primary care, and social support while optimising resources and improving quality of life.

As a proud member of the consortium, SIOG – International Society of Geriatric Oncology continues to bring its leadership and expertise in geriatric oncology to this collaborative effort, helping to shape the policies and practices that will define the future of cancer care for older adults.

With proof-of-concept work now within reach, the next phase will focus on pathways to real-world implementation and policy translation — turning evidence into action and ensuring that older adults in Europe will benefit from care that is more connected, proactive, and person-centred.
