“Our new paper in Advances, an ASTRO Journal in Radiation Oncology presents a global assessment of megavoltage radiotherapy infrastructure and projections through 2045.

The message is clear: Only 68% of patients who need radiotherapy worldwide currently have adequate access. By 2045, more than 20,000 additional machines will be required. Meanwhile, a large proportion of existing equipment is aging – even in high-income countries.

Radiotherapy is a cornerstone of cancer care. Without strategic planning, sustained investment, and global collaboration, the access gap will continue to widen. A special thank you to Reza Ghalehtaki for leading this ambitious global effort, and to all co-authors for the outstanding teamwork.

Equitable access to radiotherapy is not optional. It is a global responsibility.
