Why Is PD-L1 Testing Essential Before Initiating Treatment in Selected Patients with Advanced NSCLC?

The management of advanced non-small cell lung cancer (NSCLC) has evolved beyond histology and disease stage. Today, treatment selection is increasingly driven by tumor biology and molecular biomarkers.

PD-L1 is a predictive biomarker that helps identify patients who are most likely to benefit from first-line immune checkpoint inhibitor therapy. It measures PD-L1 expression on tumor cells, which contributes to immune evasion.

Current international guidelines recommend PD-L1 testing before initiating systemic therapy in appropriate patients with advanced NSCLC.

Clinical evidence has consistently demonstrated that patients with PD-L1 expression ≥50%, in the absence of actionable driver mutations, derive significant benefit from first-line immunotherapy compared with platinum-based chemotherapy.
