“Just out in Nature Medicine: our paper introducing the Value Cube, a three-dimensional framework for assessing value in global oncology.
About a year ago, Ophira Ginsburg, Dario Trapani and I started sketching this concept together.
In oncology, we are familiar with the gap between efficacy in clinical trials and real-world effectiveness. The Value Cube asks us to look one step further: even when a treatment works, can a particular health system actually convert that benefit into real value? A therapy may look highly promising on trial evidence, but if the system cannot identify the right patient, deliver treatment safely, manage toxicity, or sustain the financial burden, much of that value remains theoretical.
The Value Cube is designed to make that gap visible-asking not only whether an intervention works and what it costs, but what value can actually be realized in a specific health system at a given time. It is not meant to replace existing frameworks such as ESMO-MCBS or the ASCO Value Framework, but to complement them by making health-system capacity an explicit part of value assessment.
We view this paper as a starting point-a conceptual foundation to build on, rather than a finished system. Our next goal is to make the framework operational through measurable indicators, scoring, validation, and real-world testing.
To achieve this, we plan to expand the team with colleagues from different regions and health-system settings by the end of 2026, and officially begin the next phase of the project in early 2027.
With this publication, we make an open invitation to the global oncology community: please read the concept, challenge it, and help us improve it.
