“Change Is Hard in Medicine. But Standing Still Is Harder.

A recent discussion with fellow oncologists brought me back to the powerful keynote by Armando Giuliano at San Antonio Breast Cancer Symposium 2025.

His pioneering work on sentinel lymph node biopsy reshaped surgical oncology, despite prolonged rejection and criticism.

Core reflections:

  • Even the smallest change can trigger resistance
  • Comfort zones feel safer than progress
  • The fear of change is often louder than the evidence
  • Progress requires someone willing to absorb discomfort

Leading change is not about popularity. It is about responsibility.
It means staying the course, absorbing friction, and moving forward so our patients and healthcare system can benefit from progress.

No matter your title, age, or years in practice, evolution is always possible.
And necessary.

We must be bold! Because in medicine, standing still is not neutral. It is falling behind.”
