Aleix Prat: What if the Tumor is the Mountain and the Immune System is the Snow in Breast Cancer

Aleix Prat, Director of the Clínic Barcelona Comprehensive Cancer Center at Barcelona Clinical Hospital, shared a post on LinkedIn:

“What if the tumor is the mountain… and the immune system is the snow covering it?

At first glance, the snow looks homogeneous-thick, thin, present, absent.

But it’s not!

Where it accumulates, where it melts, where it exposes what lies beneath-and especially how it is organized… that carries a lot of clinically relevant information.

The immune system in patients with newly diagnosed breast cancer is much the same.

What often looks like complexity is not noise. It’s signal.

And yet, we tend to simplify it, counting TILs, reducing a structured system into a single dimension.

But the immune system is not just ‘there’ or ‘not there.’ It’s organized, dynamic, and informative, and we now have better tools to measure it properly.

With well-trained immune profiling, including HER2DX, we’ve shown it can guide us, helping predict pCR and risk, beyond what TILs alone can capture.

This is where precision oncology needs to go. Not simpler. More precise.

We are missing part of the landscape.

We’re only beginning to uncover the full power of immune profiling at diagnosis, and we’ll be sharing more key findings from our group soon. MRD captures tumor burden, but not the immune system dimension.”
