”2026 ASCO Living Guideline for Recurrent Ovarian Cancer: What is practice-changing?
- Relacorilant + nab-paclitaxel emerges as a new option for platinum-resistant disease, with both PFS and OS benefit in ROSELLA.
- FRα is moving to the center of treatment selection, while the role of PARP inhibitors in the recurrent setting is narrowing.
Key recommendations:
- Platinum-sensitive → platinum doublet ± bevacizumab
- Platinum-resistant, FRα-high (≥75% tumor cells with ≥2+ IHC staining) → mirvetuximab soravtansine
- Platinum-resistant → relacorilant + nab-paclitaxel – new ASCO-recommended option
- PARPi → increasingly selective role in recurrent disease
- Secondary cytoreduction → selected patients with a high likelihood of complete gross resection
Recurrent ovarian cancer treatment is becoming increasingly biomarker- and context-driven.”
