Aastha Gupta: Understanding Immune Shifts Helps Predict Treatment Response and Potential Risks

Aastha Gupta, Additional Director and Head of Hematopathology and Molecular Oncology at Fortis Memorial Research Institute, India, shared a post on LinkedIn:

“Extended immune subset analysis can reveal insights that routine tests often miss.

In one of our recent myelofibrosis cases on JAK inhibition (Jakavi), deep immunophenotyping showed a clear pattern: significantly reduced memory T-cell subsets with relatively preserved naïve resting T cells.

This helped explain the sustained therapeutic response to Jakavi—the drug’s action extends to immune subsets that drive chronic inflammation and disease activity. But it also highlighted a double-edged reality: suppressing memory T cells may support disease control, but at the same time it can also increase susceptibility to infections.

This case is a reminder that:

  • Immune cells influence not just the disease, but also how our therapies behave.
  • Understanding these shifts allows us to better predict treatment response and potential risks.
  • Advanced immune profiling is becoming essential for personalised, safer patient care.

The immune system doesn’t just reflect the illness—it helps tell the story of the treatment!!”
