Paola Ghanem: Our work on the druggable genomic landscape of GBM is finally out

“Our work on the druggable genomic landscape of GBM is finally out! We first find a significant proportion of targetable hotspot mutation in GBM. However, we also identify an important fraction of sub-clonal mutations compared to that of breast/lung Ca.

We aimed to identify imp biochemical features of targeted therapies and their ability to concentrate in the brain. Lipid solubility and surface area, are key features but highlight the need to consider the composite effect of multiple favorable features (see below).

In fact, there remains a significant pre-clinical and clinical gap in understanding the ability of those drugs to concentrate in brain enhancing and NON-enhancing regions, which in part explains the failure of targeted tx despite a high freq of hotspot muts.

We then explored our experience based on a real-world based cohort at the Johns Hopkins Molecular Tumor Board and highlight the encouraging clinical utility of targeted tx based on genomic characterization.

Finally, with the expanding role of Precision Oncology and the increasing accessability of NGS, efforts are needed to investigate targeted therapy in genomically-designed/driven clinical trials.
