Young Kwang Chae: Can AI-Based TLS Assessment Refine Immunotherapy Outcomes Beyond TIL Density?

Young Kwang Chae, Professor of Medicine at Northwestern University, shared on X:

“Thrilled to present our latest multi-institutional pan-cancer research at ESMO26!

Can AI–driven assessment of Tertiary Lymphoid Structures (TLS) refine immunotherapy outcomes beyond standard TIL density?

Our analysis of n=2,023 ICI-treated patients shows:

  • Added Prognostic Value: TLS presence predicts significantly longer PFS & OS, particularly within the TIL-low subgroup.
  • Beyond Lung Cancer: TLS prognostic significance remains prominent across non-lung solid tumors (n=755).
  • Dual Blockade: In anti-PD-1/CTLA-4 combination therapy, TLS presence (rather than TIL density) emerged as a key determinator of survival benefit.

Combining spatial AI quantification of TLS + TIL density via Lunit H&E analysis enables refined biomarker stratification across diverse tumor types.

Grateful to our outstanding collaborators including Dr. Yoojoo Lim!”
