With the revision of IMS IMWG high risk myeloma definition to make it stricter, and relaxation of diagnostic criteria for plasma cell leukemia to 5% PCs, the two groups have come closer in outcomes. They share a need for new ways overcome risk and improve prognosis with unified clinical trials. We also need to eliminate confusion for patients.

In our view Plasma Cell Leukemia is a form of high risk myeloma. The term captures risk the same way del17 or EMD captures risk.

High-risk multiple myeloma:

Multiple Myeloma meeting.
- International Myeloma SocietyInternational Myeloma Working.
- Group criteria for high-risk.
- Del 17p and/or TP53mutation.
- Bi-allelic del 1p.
- t(4;14), (14;16), or t(14;20) plus either Gain/Amp 1q or Del 1p.
- Gain/Amp 1q plus Del 1p.
- B2M greater than 5-5 with normal renal function.

Multiple myeloma with extramedullary disease.”
