Thor Halfdanarson: Further Evidence Against Right Hemicolectomy in Appendiceal NETs

Thor Halfdanarson, Medical Oncologist at the Mayo Clinic, shared a post on X about a paper by Swathi R. Raikot et al. published in the Journal of the American College of Surgeons:

“Further evidence against right hemicolectomy in appendiceal NETs (pediatric population). That said, 10% of appendectomy patients had nodal evaluation and of them, 16% had LN+. The endpoint of the study was 5 and 10 years OS. Still some questions remain…

While I agree 100% with not using routine right hemicolectomy, 2 major questions remain:

  1. What is the long-term (beyond 10-20 years) outcome of those with undetected nodal mets? Likely very good, but need data.
  2. Should we follow these young patients at all? If so, how frequently?”
