A New Adjuvant Option for Patients with dMMR Stage III Colon Cancer  – The Babak Lab

“A new adjuvant option for patients with dMMR Stage III colon cancer

The FDA has approved atezolizumab (Tecentriq) in combination with fluoropyrimidine and oxaliplatin chemotherapy for the adjuvant treatment of patients with Stage III mismatch repair-deficient (dMMR) colon cancer.

Key Insights

  • The approval is based on the Phase 3 ATOMIC/ML39057 trial, which included 712 patients after complete surgical resection.
  • Patients received either atezolizumab + mFOLFOX6, followed by atezolizumab alone, or mFOLFOX6 alone.
  • Adding atezolizumab reduced the risk of disease recurrence or death by 50% (HR 0.50; p = 0.0001).
  • Median disease-free survival had not been reached in either group at the time of the analysis.
  • The approval is specifically for dMMR Stage III colon cancer, making tumor MMR status an important factor in selecting patients for this treatment.

Why it matters

This approval brings an immune checkpoint inhibitor into the adjuvant treatment of a molecularly defined group of colon cancer patients. It also adds to the growing evidence that biomarkers such as MMR status can help guide treatment decisions earlier in the course of the disease.”
