Sucharu Prakash: Turning MRD Detection Into Actionable Clinical Decisions

Sucharu Prakash, Medical Director Precision Medicine at The US Oncology Network, shared on LinkedIn:

“Data keeps evolving! Brings forth new challenges! The future of precision medicine is increasingly about detecting cancer recurrence before it becomes visible on a scan. And that is incredibly exciting.

Despite major advances in early-stage lung cancer treatment, recurrence remains a significant challenge. Emerging data published in The Journal of Molecular Diagnostics highlight the potential of ctDNA and liquid biopsy to detect MRD and identify patients at risk of relapse.

This is where precision medicine has the potential to transform cancer care.

MRD and ctDNA are opening a new window into cancer biology, helping us understand what may be happening at the molecular level long before conventional imaging can reveal it.

But detecting MRD is only the beginning. The next critical question is: How should we act on this information?

As more high-quality clinical data emerge, we can work toward evidence-based, tumor-specific guidelines that help determine:

  • Escalation: Which patients with detectable MRD may benefit from earlier or more intensive treatment?
  • De-escalation: Which patients with favorable molecular results might safely avoid unnecessary treatment and its associated toxicity?
  • Monitoring: How frequently should we assess ctDNA, and how should changes over time influence clinical decisions?
  • Clinical trials: How can MRD help identify high-risk patients and accelerate the development of more effective, personalized treatment strategies?

As we continue to generate robust prospective data, our goal must be to translate molecular insights into actionable clinical guidance, improving outcomes while avoiding both undertreatment and overtreatment.

As Medical Director of Precision Medicine for The US Oncology Network and Director of Quality for Texas Oncology.

I am excited about the opportunities ahead. Our challenge is to move beyond simply identifying molecular signals to establishing when, how, and for whom those signals should change clinical management.

These are exciting times for precision oncology. The science is advancing rapidly, and the opportunity to make cancer care more personalized, proactive, and evidence-based has never been greater.”
