“Excited to share the publication of our latest study in Digital Medicine, demonstrating the potential of a digital-first approach to tackle one of healthcare’s most persistent challenges: hospital readmissions.
Among 1,713 patients with cancer, a high-risk, medically complex population, a digital transitional care management intervention was associated with significantly lower hospital readmissions at both 30 days (30% vs. 37%) and 90 days (48% vs. 54%) compared with usual care.
These findings highlight how scalable, digital-first solutions can help address some of the most complex challenges in healthcare while supporting patients during a particularly vulnerable period.
I’m incredibly grateful to our outstanding team at MSK (Jun Mao, Katherine Panageas, Deb Schrag Jennie Huang Jessie Holland, Ann M. Mazzella Ebstein Chasity Burrows Walters Jill Ackerman, Nitya Raj Rachel Sanford Martin Voss and others) and to the National Cancer Institute (NCI) for funding an R37 grant to evaluate the next generation of this approach – combining digital symptom monitoring and management with virtual integrative medicine therapies, including yoga, structured exercise, and mind-body practices, to help patients better manage symptoms and recover at home after discharge.
