Rishabh Jain: Are We Treating Metastatic Prostate Cancer Earlier, or Overtreating Patients Sooner?

Rishabh Jain, Medical Consultant at Renova Hospital (RHL), shared on X:

“Our correspondence is now published in the New England Journal of Medicine!

TALAPRO-3: Are we treating metastatic prostate cancer earlier, or overtreating patients sooner?

Talazoparib with enzalutamide improves radiographic PFS. But important questions remain:

  • Overall survival benefit remains unproven (HR 0.77).
  • Only 10% of control patients subsequently received a PARP inhibitor.
  • Grade ≥3 anemia: 51%.
  • Blood transfusions: 40%.

The key question: Is upfront PARP inhibition superior to an optimized sequential strategy?

We raised this concern in NEJM, and the TALAPRO-3 investigators, including Neeraj Agarwal and Karim Fizazi, responded.

A valuable scientific exchange on efficacy, toxicity, sequencing, and the future of precision oncology.”
