“Zongertinib has already given us a first-line targeted option for HER2-mutant NSCLC with TKD mutations. This week, AstraZeneca reported results from the phase 3 DESTINY-Lung04 trial.
Trastuzumab deruxtecan showed a statistically significant and clinically meaningful improvement in progression-free survival versus platinum/pemetrexed plus pembrolizumab in first-line advanced HER2-mutant nonsquamous NSCLC, in patients with exon 19 or exon 20 mutations.
Two things I am thinking about:
- Sequencing. With an oral TKI and an ADC both available in the first line, how do we choose? Is there a case for combining them? Dual HER2 inhibition is an obvious question, but will it be tolerable?
- Who is still left out? Extracellular domain alterations, such as S310F, account for a meaningful proportion of HER2-altered NSCLC, and neither of these first-line options was studied there.”
