Udhayvir Grewal: Formal Palliative Care is Underused and Inequitably Delivered in NETs

“Just out in Journal of the NCCN — mentoring Suriya Baskar on this project (and others) has been one of the most fulfilling experiences of my career.
We take on a rare care delivery topic relevant to the care of patients with Neuroendocrine Tumors
What we found in >7,000 inpatient deaths among patients with NETs:
40% didn’t receive formal palliative care at end-of-life
Those who did had:
Higher odds of DNR status ( OR 5.2)
Decreased Mechanical ventilation ( OR 0.47)
Decreased Vasopressors ( OR 0.70)
Decreased Blood transfusions ( OR 0.65)
Decreased Hospital stay and ~$70K decrease in charges
Disparities we uncovered:
Black patients were:
Less likely to get formal palliative care ( OR 0.70)
Less likely to have DNR status ( OR 0.81)
More likely to receive mechanical ventilation ( OR 1.54)
Takeaway: Formal palliative care is underused and inequitably delivered in NETs. We must do better to ensure compassionate, goal-aligned end-of-life care for all patients.
