“ESMO 2026 – should darolutamide join ADT + radiation in very-high-risk localized prostate cancer?

DASL-HiCaP: MFS HR 0.42, 4-year MFS 95% vs 88%.

In the phase 3 DASL-HiCaP trial (ANZUP1801, n=1,106), Nubeqa (darolutamide) 600 mg BID for 96 weeks plus LHRH analog and locoregional RT reduced the risk of metastasis or death vs placebo plus ADT + RT.

PSA-PFS HR was 0.47 at a median follow-up of 3.8 years.

Grade ≥3 AEs were 43% vs 36%.

Caveat: OS is immature (HR 0.88, not significant), and Nubeqa is not approved for localized disease with RT.

Presented as Presidential LBA4 (Oct 24, 2026); co-lead Dr. Tamim Niazi.
