“Management of Advanced Prostate Cancer? A growing area for Cancer Coaching?

Prostate Cancer Management – it used to be relatively simple decision making . As a function of personalized risk nomograms- plotting sequential individual PSA serum values versus histological Gleason grades (x+y) from prostate biopsy in the context of any given patient (age/ previous medical history/ family history)…. Etc.

Then starting with surgery (or radiotherapy) or both plus – if required upfront hormone anti testosterone or LHRH agonists as standard treatment both neo adjuvant or adjuvant -or palliative in the context of locally advanced or metastatic cancer?

Now there are so many different therapeutic options that the subject seems more like the many “breast cancer treatment options”- requiring more “Saint Gallen” meetings – like the classical Breast Cancers consensus meetings based on veteran physicians options and democratic votes ? Often leading to “No Consensus” sessions?

Report from APCCC24 – The 2024 Advanced Prostate Cancer Consensus Conference (APCCC) highlights significant advances in prostate cancer management while underscoring areas where high-quality evidence is still lacking.

The conference gathered expert opinions on controversial clinical issues, particularly in advanced prostate cancer, offering guidance in cases where guidelines are unclear or conflicting.

The report emphasizes personalized treatment approaches, considering patient-specific factors and evolving clinical data, and stresses the importance of ongoing clinical trials to fill knowledge gaps.

It provides a practical framework for physicians and patients to engage in shared decision-making, especially in complex cases of high-risk localized and locally advanced prostate cancer.

Although scientific progress has contributed to improvements in outcomes for patients with advanced prostate cancer, there are still a variety of topics that greatly impact daily practice for which high-level evidence is lacking.

The results from APCCC 2024 can help physicians and patients navigate controversial areas of clinical management for which there is no high-level evidence or the data are conflicting. APCCC 2024 identified important gaps (areas of nonconsensus) that merit evaluation in specifically designed trials.

Rationally , the decision on which therapies are most appropriate in every situation, and which precise sequence of them, depends not only on evidence from relevant specific clinical context outcomes, but also on personal patients preference and financial considerations?

Hence a good practice for the oncology expert might be to tailor explanations and honest communication to each individual patient to test for their specific preferences and circumstances? Cancer coaching is also information management and the art of medicine in any particular case?”
