A nationwide study coordinated by CCIT-DK – National Center for Cancer Immune Therapy of Denmark researchers investigated (through Danish Metastatic Melanoma Database (DAMMED)) the outcomes of 838 real-world patients diagnosed with melanoma brain metastases (MBM) or leptomeningeal disease (LMD) between 2015–2022.
Key Findings and Clinical Implications:
- Checkpoint inhibitors use associated to better survival: Patients treated with ipilimumab + nivolumab had a 2-year overall survival (OS) of 49%, reinforcing the role of immunotherapy in MBM management.
- LMD remains a challenge: Patients with LMD had limited benefit from immunotherapy, highlighting the need for new treatment approaches.
- Postoperative SRS not associated to survival benefit: Its routine use in MBM should be considered carefully.
- Long-term survival is possible: 19.4% of patients survived beyond 3 years, demonstrating progress but also the need for further advancements.
These results emphasize the growing impact of systemic therapies in MBM while underlining the urgent need for better strategies in LMD.
