“We now have overall survival data supporting TWO adjuvant CDK4/6 inhibitors in HR+/HER2− early breast cancer!
Updated phase III NATALEE results presented at ESMO – European Society for Medical Oncology 2026 demonstrate an encouraging overall survival (OS) benefit with ribociclib + NSAI.
Key Results (66.5-month median OS follow-up)
- OS: HR 0.809 (95% CI, 0.661–0.989); nominal P = .019
- 6-year OS: 91.86% vs 89.73% (2.1% absolute benefit)
- iDFS: HR 0.741 (95% CI, 0.646–0.849)
- 6-year iDFS: 83.23% vs 77.92% (5.3% absolute benefit)
Importantly, patients had been off ribociclib for a median of 38 months, suggesting durable benefit.
NATALEE had iDFS as its primary endpoint, with OS as a secondary endpoint. The reported OS P value is nominal, raising questions about formal statistical significance after accounting for multiplicity.
My takeaway
The consistency across iDFS, distant recurrence, breast cancer-specific survival, and OS is compelling. However, an encouraging OS signal is not necessarily the same as a formally established OS benefit.
These results further strengthen the role of adjuvant CDK4/6 inhibition while highlighting the importance of appropriate patient selection.”
