“Very excited to see this work published.
The combination of Cemiplimab and Fianlimab (anti-LAG-3) was very powerful and led to the highest rate of complete or near-complete response that we’ve seen In ISPY in the HER2- subtypes.
The problem is that it also increased the rate of immune related complications such as adrenal insufficiency and diabetes. Now, we need to figure out how to get that same response without triggering auto-immune responses.
The solution may be to find the people early on who are at risk for these complications in time, to keep them from having a lifelong complication. There are going to be some new and exciting ways to find an early warning that these changes are in process, hopefully in time to stop medicines while complications are still reversible.
We accomplished this using CT scans during treatment to prevent ILD in the Rilvegostomig + T-DXd combination that we presented at ASCO in June. Pulmonologist Tejaswini Kulkarni helped us find early signs of ILD and no one on that arm was symptomatic for more than a day. Amrita Basu and the whole ePRO (patient reported outcomes) working group (Elad Ziv, Zoe Quandt, Rita Nanda, Ron Cohen, Hope Rugo, Dawn Hershman, and more) are all working hard to figure this out.”
