“1st-line Osimertinib
The decision to use osimertinib monotherapy as 1st-line treatment for advanced EGFR-positive NSCLC is becoming increasingly difficult.
Let’s examine patient selection based on several cohort study papers. But it does not establish whether monotherapy should be preferred over an intensified regimen.
First, let’s look at the key findings from the Phase 3 AGAIN trial, a clinical trial conducted in Japanese patients.
The clinical trial results were negative. What’s noteworthy is that with osimertinib monotherapy, overall survival (OS) was 54 months and the 5-year survival rate was 43.9%.
These results exceeded the OS data for gefitinib by more than one year.

Next is a clinical study that identified factors influencing the long-term efficacy of osimertinib. In the group with higher scores, OS was 68.3 months.

This paper is a systematic review and meta-analysis. L858R, ECOG PS, and CNS metastases were identified as factors associated with poor prognosis.
Finally, a paper presenting the results obtained after adjusting for confounding factors using propensity score matching (PSM).”
