“New in Journal of Hepatology: Is ‘less is more’ the key to combining TACE with systemic therapy in HCC?
- TACE leaves a hypoxic, structurally immature vascular niche driven by PDGFC⁺ TAMs, fueling immunosuppression and pericyte-to-CAF transition.
- Low-dose Bevacizumab + Atezolizumab with TACE promotes vascular normalization, boosts CD8⁺ T cells, and improves survival vs. high-dose, while preserving hepatic function:
– PFS: 9.73 vs. 7.43 mo (HR 0.66, P=0.036)
– OS: 31.4 vs. 21.3 mo (HR 0.58, P=0.022)”
