“Does pushing prostate SBRT past 40 Gy cost the patient anything?
In 872 men, clinically meaningful late urinary worsening ran 80% above 40 Gy versus 68% at or below.
The study:
- Single-institution retrospective cohort, 940 men on five-fraction prostate SBRT; 872 with PROs.
- 200 got more than 40 Gy, 672 got 40 Gy or less. Primary outcomes: EPIC urinary and bowel.
What it showed:
- Both groups declined acutely, then recovered early.
- Late urinary trajectories were less favorable above 40 Gy, P < 0.001.
- MCID late urinary worsening 80% versus 68%, P = 0.007.
The point:
- Escalation above 40 Gy is not free on urinary QoL.
- Dose was not randomized; it followed practice over eight years.
- Late urinary worsening was common in both arms, which nobody quotes.
