Gilberto Lopes: Key Insights from ESMO 2026

The ESMO Congress 2026 is the annual meeting of the European Society for Medical Oncology. It stands as one of the world’s most influential oncology platforms for presenting pioneering cancer research. The event takes place from October 23 to 27, 2026, in Madrid, Spain, at the IFEMA MADRID convention center, utilizing a highly accessible hybrid (in-person and virtual) format.

Gilberto Lopes, Chief of the Division of Medical Oncology at the Sylvester Comprehensive Cancer Center, shared on X:

“Major KRAS G12C NSCLC results at ESMO26!

Phase III Krascendo 1 (N=338): divarasib vs sotorasib/adagrasib.

  • OS: 17.7 vs 12.0 mo
  • HR 0.56 (95% CI 0.41–0.78)
  • PFS: 9.8 vs 5.7 mo
  • HR 0.58 (0.44–0.76)

An important randomized survival advance.

  • ORR: 58.4% vs 32.3%
  • DOR: 11.9 vs 6.9 mo

Serious TRAEs: 7.8% vs 15.2%
TRAEs leading to discontinuation: 4.8% vs 11.7%

Gilberto Lopes

Caveats: few adagrasib-treated patients, untreated brain metastases excluded, limited follow-up. Divarasib remains investigational.

Next: CNS efficacy, resistance, sequencing and moving KRAS inhibition earlier.

Skoulidis et al. NEJM 2026.”

SAFFRON Trial Shows OS Benefit With Osimertinib + Savolitinib in EGFR-Mutated NSCLC

“Important EGFR-mutated NSCLC results at ESMO26!

Phase III SAFFRON (N=338):

Osimertinib + savolitinib vs platinum – pemetrexed after osimertinib progression, with high MET overexpression and/or amplification.

  • OS: 17.8 vs 12.9 mo
  • HR 0.68 (95% CI 0.50-0.91)
  • PFS: 7.6 vs 5.4 mo
  • HR 0.55 (0.42-0.72)

Gilberto Lopes

Additional SAFFRON results:

  • ORR: 51% vs 27%
  • 12-mo OS: 70% vs 53%

MET-amplified subgroup:
PFS: 7.6 vs 4.2 mo
HR 0.487 (0.35-0.66)

Subgroup not formally tested. Grade ≥3 TEAEs: 64% vs 63%.

Key message: Test for MET-mediated resistance. Adding MET inhibition while maintaining EGFR suppression improves survival in biomarker-selected disease. Next: sequencing, CNS outcomes, and access.

US employer-sponsored family health insurance now costs $28,303/year.

KFF 2026:

  • Worker contributions: +9%
  • Total premiums: +5%
  • Wages: +3.7%
  • Inflation: +2.7%

Shifting costs to workers isn’t the same as reducing healthcare costs.”
