A timely debate with real consequences for patients. Enthusiasm for immunotherapy is justified, but no randomized trial has demonstrated that it can safely replace pelvic radiotherapy in completely resected stage III dMMR disease.
- PORTEC-3 included radiotherapy in both arms
- GOG-258 improved locoregional control with chemoradiotherapy but did not include immunotherapy
- KEYNOTE-B21 did not randomize radiotherapy.
Our message:
- integrate rather than omit.
- Treatment de-escalation requires direct evidence
- Locoregional control, recurrence burden, and quality of life matter alongside survival
What evidence would you require before omitting pelvic radiotherapy in this setting?”
