Safer Cancer Care with Blinatumomab for Children With B-ALL – ecancer

Blinatumomab may offer a safer and more effective alternative to highly intensive chemotherapy for children with newly diagnosed high-risk B-cell acute lymphoblastic leukaemia (ALL).

In this ecancer interview from EHA2026, Martin Schrappe (University Hospital Schleswig-Holstein), discusses results from a randomised trial evaluating two cycles of blinatumomab as a replacement for conventional intensive chemotherapy blocks.

Key findings include:

  • 4-year event-free survival of 83% with blinatumomab versus 70% with standard chemotherapy
  • Lower relapse rates, including central nervous system relapse
  • Substantially improved minimal residual disease responses
  • Fewer severe toxicities, including reduced infections, mucositis and treatment-related mortality
  • A modest increase in manageable neurological events

These results suggest that replacing highly intensive chemotherapy with blinatumomab can improve outcomes while reducing treatment-related toxicity.

The findings support a shift towards immunotherapy-based treatment strategies for children with high-risk B-ALL.
