“Does more venetoclax exposure necessarily mean more efficacy in AML?
Happy to share our recent publication in Leukemia and Lymphoma: Comparable response rates of venetoclax 50 mg with posaconazole versus venetoclax 400 mg despite lower pharmacokinetic exposure in newly diagnosed AML: less is enough.
In this prospective study, VEN 50 mg with posaconazole produced significantly lower drug exposure than VEN 400 mg, yet clinical responses remained comparable: CRc 60% vs 63.6%, with similar MRD-negative rates.
We also observed faster neutrophil recovery with the lower-dose strategy, raising an important question about whether higher venetoclax exposure always translates into greater clinical benefit.
These findings support further exploration of more individualized venetoclax dosing, particularly when strong CYP3A inhibitors are used.
