BioNTech announced that its Phase 2/3 AHEAD-MERIT trial did not meet the progression-free survival (PFS) endpoint, one of the study’s two primary endpoints. The trial is evaluating the investigational mRNA cancer immunotherapy BNT113, also known as tenumomeran/raplemomeran, in combination with pembrolizumab in patients with HPV16-positive head and neck cancer.
At the first topline analysis, BNT113 plus pembrolizumab did not demonstrate a statistically significant improvement in PFS compared with pembrolizumab alone. The study will continue as planned to evaluate its second primary endpoint, overall survival (OS), following a recommendation from the Independent Data Monitoring Committee.
The AHEAD-MERIT trial is investigating the combination as a first-line treatment for patients with unresectable recurrent or metastatic HPV16-positive head and neck squamous cell carcinoma (HNSCC) expressing PD-L1.
Overall Survival Remains Central to the Trial’s Assessment
The Phase 3 component of AHEAD-MERIT evaluates PFS and OS as complementary measures of clinical benefit. Although the combination did not meet the PFS endpoint, the trial’s prespecified statistical design allows for a positive outcome if a statistically significant improvement in overall survival is demonstrated.
The Independent Data Monitoring Committee recommended continuing the study to complete the assessment of whether BNT113 plus pembrolizumab can help patients live longer. BioNTech remains blinded to ongoing trial data in accordance with the study’s governance procedures.
Prof. Özlem Türeci, Co-Founder and Chief Medical Officer at BioNTech, addressed the findings and the rationale for continuing the trial:
“The potential of mRNA cancer immunotherapies in combination regimen lies in their ability to induce immune responses that may translate into durable clinical benefit and, ultimately, prolonged survival of patients. While the PFS result was not statistically significant at this first analysis, the data support continued evaluation of BNT113 plus pembrolizumab in this trial and show that it is engaging its intended therapeutic mechanism.”
Türeci added that immune-mediated antitumor effects may build and deepen over time, making the overall survival analysis critical to assessing the approach’s potential to deliver durable benefit in HPV-driven head and neck cancer.
From Safety Evaluation to Randomized Comparison
AHEAD-MERIT is an open-label, controlled, multisite, two-arm Phase 2/3 trial comparing BNT113 plus pembrolizumab with pembrolizumab monotherapy. Eligible patients have unresectable recurrent or metastatic HPV16-positive HNSCC with PD-L1 expression at a combined positive score (CPS) of at least 1.
The study includes two parts with distinct objectives:
- Part A is an initial, nonrandomized safety run-in designed to confirm the safety and tolerability of BNT113 in combination with pembrolizumab at the selected dose range.
- Part B is randomized and designed to generate pivotal efficacy and safety data by comparing the combination with pembrolizumab alone in the first-line setting.
Patients receive their assigned treatment for up to approximately 24 months. Alongside the two primary endpoints of PFS and OS, secondary endpoints include safety, objective response rate and duration of response.
BNT113 Targets Two HPV16 Proteins
BNT113 is an investigational mRNA cancer immunotherapy that encodes E6 and E7, two HPV16-derived proteins frequently found in HPV16-positive solid tumors.
The candidate uses a fixed set of antigens and is designed to induce HPV16-specific antitumor immune responses. In combination with checkpoint inhibitor treatment, the approach aims to enhance clinical responses by directing immune activity against these tumor-associated viral proteins.
BNT113 received Fast Track designation from the US Food and Drug Administration in January 2026. Its evaluation in AHEAD-MERIT continues, with overall survival remaining under assessment.
Understanding Head and Neck Cancer and the Role of HPV
Head and neck cancers encompass malignancies arising in the mouth, throat, voice box, nasal cavity, sinuses and salivary glands. Most begin in the squamous cells lining the region’s mucosal surfaces and are classified as head and neck squamous cell carcinoma (HNSCC).
Human papillomavirus, particularly HPV16, is a major cause of oropharyngeal cancer, which affects areas such as the tonsils and base of the tongue. This association is particularly relevant to research targeting HPV-driven tumors.Symptoms can include a persistent neck lump, a mouth or throat sore that does not heal, difficulty swallowing, persistent hoarseness or voice changes.
Treatment depends on the tumor’s location, stage and the patient’s overall health, and may include surgery, radiotherapy, chemotherapy, targeted therapy or immunotherapy.
The Need for HPV-Targeted Treatment Options
According to BioNTech’s release, approximately one-third of HNSCC cases are associated with HPV infection, with the majority linked to HPV16, a high-risk HPV type associated with cancer. Despite the distinct characteristics of HPV-positive tumors, the company noted that there are currently no approved HPV-targeted treatments. Many patients with HPV16-positive HNSCC experience disease progression while receiving current standard-of-care therapies.
BNT113 is being developed to address this treatment need by targeting HPV16-derived antigens expressed by tumor cells. The ongoing trial is assessing whether adding this approach to pembrolizumab can improve outcomes compared with pembrolizumab alone.
mRNA Immunotherapies Within BioNTech’s Oncology Pipeline
BioNTech describes mRNA cancer immunotherapies as a key component of its oncology strategy, alongside immunomodulators and antibody-drug conjugates. Its development approach includes treatment combinations intended to address cancer through complementary mechanisms.
For BNT113, the next assessment within AHEAD-MERIT is the second primary endpoint of overall survival. That analysis will complete the trial’s evaluation of the combination’s potential to prolong survival in patients with HPV16-positive head and neck cancer.
