Beyond individual trial results, several broader themes emerged that may influence how clinicians approach daily practice. Here are five key messages from ESMO GI 2026 that could shape multidisciplinary decision-making in the coming years.
1. Precision Oncology Is Moving Earlier
Comprehensive molecular profiling is no longer an optional step reserved for patients who have exhausted standard therapies. Instead, biomarker assessment should be integrated at diagnosis whenever feasible.
In advanced gastric and gastroesophageal junction cancers, HER2 expression, PD-L1 CPS, MSI status, and Claudin 18.2 have become important components of treatment planning. Similarly, metastatic colorectal cancer management increasingly relies on extended RAS/BRAF testing, MSI status, HER2 amplification, and the identification of rare but actionable alterations such as NTRK fusions.

Real-world evidence has become increasingly valuable for evaluating treatment effectiveness in elderly patients, frail individuals, those with multiple comorbidities, or patients receiving later-line therapies.
Large observational cohorts and prospective registries are helping clinicians understand how innovative treatments perform outside the controlled environment of clinical trials, providing complementary evidence that supports personalized treatment decisions.
