The 18th BGICC Breast & Gynecological International Cancer Conference (BGICC 2026) is taking place today, January 22, 2026, in Cairo, Egypt. “Terrific session at hashtag#BGIICC on AI in Healthcare in Cairo, Egypt. Moderated by Denis Horgan, Ph.D.

Issues discussed included platforms, endpoints for payers, bias, standardization & certification, resources in LMIC, misinformation, importance for users, i.e. physicians to have a voice to avoid issues and lessons learned with EHR’s, need for aligning training with AI innovation, need to accelerate diagnosis, e.g. of cervical cancer prelesions. Issues were raised about governance structures in AI implementation & international guidance.

I appreciated report by Dr. Julie Gralow about the American Society of Clinical Oncology (ASCO) AI Taskforce for ethical use. ASCO has identified 6 important areas that she discussed:

1- Transparency: how models are trained, inclusion of different populations such as in Africa that is relevant today,

2- Need to inform stakeholders: patients & clinicians need to know how AI is being used for example in insurance denials & emerging strategies for its use in appeals,

3- Equity/fairness: by example ensuring that algorithms do not bias against patients who did not show up to a clinic appointment because they were seen in the ER and another appointment was scheduled,

4- Accountability: Legal, regulatory, ethics. Healthcare providers need to be involved.

5- Patient privacy: Patients need to be aware that the questions they ask Chat-GPT are in the cloud.

6- Human-centered applications. Human involvement is a fundamental element of care. She spoke of strategies for having accurate evidence-based information in AI algorithms. She gave examples of use of AI to improve screening such as colonoscopies with appointments scheduled after AI discusses with patients.

The charge from Dr. Hesham ElGhazaly was to provide outcomes highlighting priority themes and coordination mechanisms.

My suggestions were:

1- Need to address data sources used for training AI with attention to the diversity within populations and the known issue of data aggregation.

2- Research is very important. I mentioned the WIN Consortium in cancer personalized medicine data science committee chaired by Dr. Gerald Batist and Dr. Yves Lussier that has been meeting for a couple of years. Challenges were identified with data interoperability, & opportunities for international collaboration, e.g. therapeutic outcomes across globe with targeting DNA repair /PARP inhibitors, immunotherapy among others.

3- There is need for prospective validation research.

4- I discussed concept WIN has developed >2 yrs ago in collaboration with Caris Life Sciences & Dr. David Spetzler that we refer to as the AI-EE (artificial intelligence for enrichment and equity) trial. This uses AI algorithms & digital pathology images to enrich for driver mutations for which there are therapeutics. Discussions of implementation are ongoing including discussions with pharma.

5- Cautionary note: Be aware with AI of slippery slope to save money. Very important.”
