Premal Patel: Beyond Biomarkers, Elephas Platform Predicts Immunotherapy Response Using Live Patient Tumor Tissue

Premal Patel, Chief Medical Officer at Elephas, shared on LinkedIn:

“Excited to share our latest publication in Cell Reports Medicine!

Immune checkpoint inhibitors have transformed cancer treatment, yet predicting which patients will benefit remains a major challenge.

Our study demonstrates the promise of functional precision medicine – moving beyond static biomarkers to directly measure how live patient tumor tissue responds to immunotherapy ex vivo. Using the Elephas elive platform, we observed 93% concordance with clinical outcomes in an initial clinical cohort.

These findings represent an important step toward more personalized advancement of immunotherapy.

Congratulations to the entire team and our collaborators!”

Interesting findings from this study:

  • Identified potential responders missed by conventional biomarkers: The platform detected patients who may benefit from immune checkpoint inhibitors despite testing negative for established predictive biomarkers.
  • Developed a simplified eight-cytokine classifier: The elive index uses changes in eight cytokines to classify tumors as likely responders or nonresponders, achieving an area under the curve of 0.99 for distinguishing the response groups identified by clustering.
  • Enabled testing with routine clinical biopsies: The platform was developed using live tumor fragments from standard-of-care biopsies, offering a potential alternative to approaches that require larger surgical tumor samples.
  • Demonstrated potential to identify objective responders: In the clinical cohort, the platform correctly identified 10 of 12 patients who had an objective response to immunotherapy, suggesting potential value in selecting patients for treatment
