“Can radiotherapy be safely delivered alongside antibody – drug conjugates (ADCs)?
The emerging evidence is reassuring.
Radiotherapy appears to be safely delivered alongside ADC therapy in most clinical settings, with severe complications occurring infrequently.
For conventional extracranial RT, current evidence supports treatment during T-DM1, T-DXd and sacituzumab govitecan (SG), provided routine safety monitoring is maintained.
The key area requiring greater caution is intracranial stereotactic radiotherapy.
In particular, patients receiving T-DM1 appear to have an increased risk of radiation necrosis following stereotactic RT – a signal that has been reported consistently across the available evidence.
Importantly, current data do not suggest the same radiation-necrosis signal with T-DXd or SG.
As ADCs become increasingly integrated into cancer treatment, understanding how best to sequence and combine them with radiotherapy will be essential for multidisciplinary practice.”
