Lung cancer care has changed. The cardiovascular questions surrounding it have changed with it.
With immune checkpoint inhibitors, thoracic radiotherapy, and increasingly complex targeted therapies reshaping treatment pathways, cardiovascular risk in lung cancer can no longer be viewed through the lens of a single toxicity. The clinical phenotype reflects an interaction between the patient, the cancer, and the treatment.
ICI myocarditis may be uncommon yet potentially severe. Thoracic radiation can affect multiple cardiac structures. Targeted and systemic therapies introduce distinct electrical, vascular, and myocardial risks. And all of this occurs in a population that may already carry substantial cardiovascular vulnerability. This is where cardiac imaging matters – but not as a routine response to a treatment label.
Its real value lies in answering the clinical question:
- What is the suspected mechanism?
- What cardiovascular phenotype are we looking for?
- And which imaging modality can meaningfully change the next clinical decision?
In this Editorial Perspective, following World Lung Cancer Day, we look beyond the lung to examine the evolving cardiovascular landscape of modern lung cancer care from immune checkpoint myocarditis to radiation-related cardiac injury, and the role of multimodality imaging in navigating both.

The goal of contemporary cardio-oncology is not simply to detect cardiovascular abnormalities, but to recognize clinically meaningful toxicity early enough to protect cardiovascular health without unnecessarily compromising effective cancer therapy.
