Andrius Žučenka: As a bone marrow transplanter I trully find BMT as one the most difficult and somewhat miraculous procedures

Andrius Žučenka, Hematologist, Researcher, Lecturer and PhD Candidate at Vilnius University Hospital Santaros Klinikos, shared on X/Twitter:

“As a bone marrow transplanter I trully find BMT as one the most difficult and somewhat miraculous procedures: A 56F AML Pt with highly proliferative R/R NPM1 and FLT3-TKD AML, refractory to 3 prior therapies (7+3 and HDAraC and Dauno/Gilteritinib and ACTIVE/Revumenib) with extramedullary.

Involvement (skin, cornea, soft tissues) and sinonasal and CNS mucormycosis, requiring extensive, radical surgery + intensive antifungal therapy. Nevertheless, we still proceeded with the sequential alloSCT: using LDAraC, Decitabine, Venetoclax, Navitoclax, Cladribine as 10-day.

Preconditioning, directly followed by FluBu MAC and matched related donor transplant. Prior to the salvage BMT the patient had 0-0.1 ANC count for more than 2 months. And how happy I was to see that on Day 16 she already had a complete ANC recovery. BM now with full donor.

Chimerism and negative MRD, no signs of extramedullar leukemia. An extremelly difficult case, ant lots of work still awaits to balance the GVL/GVHD as much as possible to have maximum antileukemic efficacy, at the same time dealing with all the BMT related complications.

BMT seemed as a sci-fi movie 12 years ago being a student. I still have the same feeling Sending every BMT transplanter best wishes and a great EBMT24 congress! I had to skip due to busy schedule and the limited number of conferences for a young parent.
