Youssef Soliman: An EGD is not ‘just’ a simple EGD

Youssef Soliman, Gastroenterologist, Clinical Assistant Professor at City of Hope, shared a post on LinkedIn:

“An EGD is not ‘just’ a simple EGD…

“We NEED a good endoscopy.”

Dr. Daniela Molena and her team at Memorial Sloan Kettering emphasized the need for good quality endoscopy (with documentation) for patients with esophageal cancer at last year’s (2024) Annual Advances and Innovations in Endoscopic Oncology and Multidisciplinary Gastrointestinal Cancer Care.

Why? Because it is critical in informing the surgical oncologist (and multidisciplinary team) on how to strategize and prioritize treatment modality.

What are those quality indicators (QIs) for EGDs?

1. Clear documentation of GEJ location
2. Barrett esophagus documentation
3. Barret esophagus Prague classification
4. Tumor proximal margin
5. Tumor distal margin
6. Luminal obstruction
7. Circumferential extension
8. Gastric extension
9. Retroflexion
10. Hiatal hernia with measurements
11. Biopsies

Looks simple enough and intuitive, right?

Dr. Molena and team analyzed the percentage of good quality EGD reports (those with >6 QIs) referred to MSK.

In patients who underwent EGD at an outside facility, and had repeat EGD at MSK, 32% of EGDs were good quality from outside facility VS 68% from MSK (p<0.001).

EUS fared better with comparable good quality reports (71% at outside facility VS 72% at MSK).

Likely reason for discrepancy in quality? The undifferentiated patient undergoing EGD at an outside facility who is found to have cancer VS EUS (or repeat EGD at a referral center) with advanced knowledge of specific indication.
