“Why Financial Toxicity (FinTox) matters in cancer care.
Key insights from Dr Fumiko Ladd Chino’s excellent plenary at MASCC25:
- It erodes quality of life and social functioning.
- Leads to borrowing, reduced spending on basics and leisure; the burden worsens over time.
- Increases bankruptcy risk; post-cancer bankruptcy is linked to a mortality risk approaching 80%.
- Even in publicly funded systems, job loss and indirect costs contribute to FinTox.
- In LMICs, key drivers include out-of-pocket payments, multiple chemotherapy visits, large household size, and treatment at private hospitals.
What can we do about FinTox? Solutions must be individual, interpersonal, and systemic:
- Screen for financial toxicity early and often.
- Refer to ‘Choosing Wisely’ by ASCO, ASTRO, SSO.
- Integrate early palliative care.
- Provide patient education and financial counseling.
- Improve postgraduate medical training on FinTox.
- Offer practical support: food, transport, parking.
- Use financial navigators or app-based tools.
- Establish FinTox Tumor Boards.”
