Liang Cheng: Is Axillary Lymph Node Dissection Needed?

Liang Cheng, Vice Chair for Translational Research at Warren Alpert Medical School of Brown University, recently shared an article he and his colleagues authored on LinkedIn:

“Just published in the February 2025 issue of Clinical Breast Cancer.

Congratulations to Professor Yihong Wang and colleagues from The Warren Alpert Medical School of Brown University.

In our study of 224 node-positive breast cancer patients treated with neoadjuvant chemotherapy, we found that axillary and locoregional recurrences were rare (3.4%) in a selected group of yN0 patients who underwent SLNB and/or ALND.

Additionally, SLNB with fewer than two positive nodes rarely resulted in a significantly different yield of positive non-SLNs or impacted short-term oncologic outcomes compared to ALND. These findings suggest that de-escalation of axillary surgery to SLNB alone may be a safe option in this patient population.
