First slide is on diagnosis.
A rectal cancer protocol MRI is critical, and here are the key features that help determine management. We use rectal gel which can help delineate intraluminal tumor.
I break up locally advanced rectal cancer into “lower risk” & “high risk.” Here is lower risk.
Most patients getting surgery do not need RT (can get upfront surg or chemotherapy). But if organ preservation intent, go with chemoRT.
The studies you should know are in red.
Now for higher risk tumors (see slide for what makes higher risk).
The SOC is TNT with long course chemoRT and chemotherapy. I still DO think there is a role for short course though, and I explain the scenarios.”

1) organ preservation,
2) limited M1 disease, and
