Healthcare in Action now has 18 teams that serve patients in multiple counties across Southern and Northern California.

We welcome inquiries from health plans and municipalities that are interested in partnering to provide healthcare services to their unhoused populations in their geographies.”

Sachin Jain is the President and CEO of SCAN Group and Health Plan and an Adjunct Professor of Medicine at Stanford University School of Medicine.
