“Happy to share in print The Oncologist– Robust response to Pembrolizumab in a patient with metastatic VIPoma with acquired TMB-high/MSI-high phenotype post treatment with temozolomide.

Bottom line- in a patient with pancreatic or lung NET post exposure to temozolomide: always check liquid biopsy (or tissue if possible) and test for a hypermutated phenotype. In the presence of MSI-H disease or co-occurring DDR alterations, response to ICI can be durable and robust.
