“The 2026 radiation oncology changes aren’t just a coding update — they represent a stacked reimbursement contraction driven by new delivery codes, reduced RVUs, loss of technical IGRT, a lower conversion factor, and restrictive eviCore policies. When modeled together, IMRT drops >20% and 3D declines as well, hitting freestanding centers especially hard.
If we want to protect access to care, now is the time for collective advocacy. Every radiation oncologist should submit feedback to eviCore and Cigna on their 2026 draft policies — silence is approval, and we cannot afford that.
