- Proton therapy (PT) in 129 ACC pts (both postop and definitive setting)
- median follow-up 3 years
- PT usually for postop R0 – R1 treatment; alternative treatment to carbon ion RT (CIRT) in unresectable/R2 pts deemed to be at major risk of toxicity
2 – , 3 – , and 5-year survival rates:
LC : 92.4 %, 90.8 % and 84 %
DFS: 77.5 %, 76.4 % and 64 %
DMFS 77.4 %, 76.3 % and 66.6 %
OS: 94.2 %, 89.4 %, and 86.7 %
Favourable acute and late toxicity profile.
Prognostic factors
- Worse DFS and DMFS in advanced T stage and solid histology
- Better LC and DFS in R0/R1 versus unresected patient plus similar LC rates for R2 and unresected patients
Highlights
- need for close follow-up and treatment intensification for advanced cases/solid hisology (e.g. higher PT tumor doses or CIRT)
- referral high expertise centers is essential
- potentially R2 invasive/mutilating surgery might be avoided, offering instead particle therapy”
