Rupam Manna: Why Cetuximab is Given 1 Week Prior to CCRT with a Loading Dose?

Rupam Manna, Medical Oncologist, Entrepreneur, interested in Metabolism education and brain science, shared a post on X:

“Why is cetuximab started 1 week before chemoradiotherapy with a loading dose?

The answer lies in EGFR biology and pharmacokinetics.

  • Loading dose (400 mg/m²): Rapidly achieves therapeutic serum levels and high EGFR receptor occupancy.
  • 1-week lead-in: Allows sustained EGFR blockade before the first fraction of radiotherapy, maximizing radiosensitization from Day 1.
  • Maintenance dose (250 mg/m² weekly): Maintains effective receptor saturation throughout the course of radiotherapy.

This schedule, established in pivotal trials, optimizes the biological effect of cetuximab during treatment for locally advanced head and neck squamous cell carcinoma.”
