“Neoadjuvant Radiotherapy Improves Local Control in Retroperitoneal Well-Differentiated Liposarcoma.

Local recurrence remains the main clinical challenge in primary retroperitoneal well-differentiated liposarcoma (WDLPS), a disease with minimal metastatic potential but significant morbidity from local progression.

A large international multicentre analysis from the TARPSWG collaboration evaluated the impact of neoadjuvant radiotherapy (RT) using propensity score matching.

Study Design:

  • Multicentre cohort (24 sarcoma centres).
  • 582 patients with primary retroperitoneal WDLPS.
  • Propensity score–matched analysis (1:2).
  • Median follow-up: 73 months.

Comparison:

  • Surgery alone.
  • Neoadjuvant RT (≈50 Gy) + surgery.

Key Results
– Significant reduction in local recurrence Isolated local recurrence:

  • RT + surgery: 10%.
  • Surgery alone: 28% (P = 0.003).
