Paolo Tarantino, Research Fellow Dana-Farber Cancer Institute and Harvard Medical Schoolpost, shared a post by Mali Barbi, Medical Oncologist at Northwell Health,  on X, adding:

A challenge is that we have no ideal neoadj regimen:

  • carbo in TCHP may not improve pCR
  • no mature EFS with THP alone
  • T-DXd-THP highly active but not powered on EFS

My recommended approach is depicted in this figure.

More nuance in our upcoming podcast.

Paolo Tarantino: T-DXd is Now FDA-Approved Neoadjuvantly

Quoting Mali Barbi‘s post:

Tumor Board Dilemma:

T-DXd is now FDA-approved neoadjuvantly (DB-11).

It drives high pCR, but SATEEN/A3 confirmed if they recur, you prob face pan-TOPO1 resistance with zero salvage.

Where are you actually drawing the line for early-stage HER2+ breast cancer today?”
