“Maintenance is a marathon, not a sprint. As we extend therapy to deepen and sustain disease control in myeloma, toxicity becomes part of the efficacy equation. ENDURANCE reminds us that indefinite maintenance may increase burdens such as fatigue, diarrhoea and neutropenia, even without an apparent increase in second primary malignancies.
The future of maintenance is not only about how long we can treat, but how long patients can live well on treatment.”
