“Neuro-Oncology Trial Spotlight:

Can re-irradiation improve survival in patients with recurrent glioblastoma? The ongoing pragmatic and EU-funded LEGATO trial aims to answer this relevant clinical question.

Study Title:

  •  Lomustine with or without reirradiation for first progression of glioblastoma (LEGATO, EORTC-2227-BTG).

Study Status:

  • Actively accruing
  • More than 230 patients enrolled

Study Design:

  • Randomized phase III trial (1:1)
  • Planned enrollment: 411 patients
  • More than 40 sites in 11 countries across Europe, expansion ongoing (contact us if you want to contribute!)

Treatment Arms:

  • Lomustine (110 mg/m² every 6 weeks)
  • Lomustine (110 mg/m² every 6 weeks) plus radiotherapy (35 Gy in 10 fractions)

Main Eligibility Criteria:

  • Histologically confirmed glioblastoma, IDH wild-type (WHO 2021)
  • First progression ≥6 months after the end of prior radiotherapy
  • Radiologically measurable disease per RANO criteria, max. tumor diameter 5 cm
  • WHO performance status of 0-2

Primary Endpoint:

  • Overall survival

Secondary Endpoints:

  • Progression-free survival
  • Response rate
  • Neurocognitive function
  • Health-related quality of life
  • Health economic parameters

Trial Registration:

Study Coordination:

  • Principle investigator: Matthias Preusser
  • Study co-coordinators: Giuseppe Minniti, Thomas Kazda
  • Trial sponsor: EORTC – European Organisation for Research and Treatment of Cancer

Trial Funding:

  • LEGATO has received funding from the European Union’s HORIZON-MISS-CANCER-2022-01 under grant agreement Nº (101103655).
